JOB SUMMARY
Under the supervision of a higher classified employee, the Community Health Worker (CHW) will perform a variety of outreach and customer service activities in both an office and non-office setting. Additionally the CHW may provide support to all divisions within the Health Department.
EXAMPLES OF DUTIES
Essential Duties:

  • Provides client-centered support for the programs in the Washtenaw County Health Department in a community, home or clinical setting. Some examples may include: community lead testing, direct observed therapy, program enrollment and other support as needed.
  • Provides community education and outreach for the health department services.
  • Develops relationships with community partners to support the values outlined in the department's strategic plan.
  • Provides clerical support as needed.
  • Provides services in a culturally competent manner. Examples may include: providing written and spoken materials in clients preferred language and utilization of interpretation services.
  • Participates in individual, program and department process reviews using a continuous quality improvement model.
  • Provides timely and accurate documentation for services provided.
  • Updates and maintains a list of community services and agencies for referral purposes.
  • Performs related duties as assigned.

EMPLOYMENT QUALIFICATIONS
Knowledge, Skills and Abilities:

  • Ability to understand and apply an array of department and county policies, procedures, rules and regulations.
  • Ability to organize, prioritize and coordinate work with minimal supervision.
  • Ability to document interactions using written communication with clear English and proper grammar, spelling and punctuation.
  • Ability to use a wide variety of computer software including, but not limited to, Microsoft Office products, Outlook and social media such as Facebook and Twitter. May receive additional training on state and department required systems.
  • Ability to work cooperatively in a team and/or community setting while establishing and maintaining effective working relationships.
  • Ability to provide quality client centered services under changing and challenging circumstances in an accepting and non-judgmental way.
  • Ability to maintain client confidentiality.
DESIRED QUALIFICATIONS
  • Applicants with a Community Health Worker Certification will receive priority.
  • Proficiency in a second language
LICENSES AND CERTIFICATIONS
  • Possession of a valid Michigan Driver's license
  • Availability of automobile, van, or truck.
PHYSICAL DEMANDS
Duties require sufficient mobility to work in a both a normal office setting and in the community. Use of standard office equipment including as well as specialized devices to communicate with hearing and speech impaired clients. Requires ability to read printed material and must be hearing and speech sufficient to communicate in person or over the phone. Requires ability to drive to multiple work locations.

EDUCATION
Possession of a high school diploma or equivalent
EXPERIENCE
One (1) year of customer service experience required.
