JOB SUMMARY
Works under the supervision of a higher classified employee to direct and coordinate program activities. Plans and oversee program operations. Serves as community resource and program liaison regarding Clinic Operations, Community Outreach, or Communicable Disease. Serves as public health nurse as assigned.

EXAMPLES OF DUTIES
Essential Duties for all coordinators:

  • Guides and coordinate the department┐s Nursing activities
  • Participates in the development of program work plan, output measurements, and monitoring progress
  • Develops program policies, procedures and guidelines in compliance with federal, state, regional and local recommendations, requirements, and regulations.
  • Participate in state and national accreditation programs and activities
  • Directs the implementation of appropriate activities, which may include; disease surveillance, case investigation, clinic activities, managing nursing students, community outreach and working with community providers/organizations.
  • Developing clinic schedules, assuring appropriate vaccine storage and handling, and ordering vaccines.
  • Conducts site visits with VFC providers to assure compliance with program requirements.
  • Develops immunization education materials for both the general public and for professional health care practitioners. Develops immunization education materials for both the general public and for professional health care practitioners.
  • Provides orientation, clinical updates and professional continuing education opportunities, to the staff.
  • Provides consultation regarding immunizations, waiver education and vaccine storage and handling.
  • Provides leadership for immunization coalitions and provider networks.

  • Monitors all aspects of program operations.
  • Completes all required reporting.
  • Performs the duties of a Public Health Nurse as workloads, temporary absences or necessity dictates.

EMPLOYMENT QUALIFICATIONS
Knowledge, skills and abilities for all coordinators:

  • Knowledge of the principles and practice of public health nursing
  • Applies theory and scope of practice of public health nursing to professional practice; knowledge of public health laws;
  • Knowledge of the functions of other health and social agencies and their relationships to public health program
  • Ability to apply ethical principles
  • Ability to identify, interpret, and implement public health laws, regulations, and policies related to specific programs;

Community Outreach Coordinator

  • Develop and implement a community health program for nursing students and provide continual oversight.
  • Conduct site visits for VFC providers, urgent cares and private providers who see high risk clients.
  • Provide oversight to school immunization program. Direct staff in school reporting activities
  • Participate in immunization Nurse Education (INE) program
Clinic Coordinator

  • Provide nursing oversight to all clinical services (TB, Wise Choice, Sexual Health, WIC and Immunization).
  • Provide oversight to the Vaccine Preventable Disease Program and conduct Immunization Action plan activities.
  • Provide oversight to lead testing program.
  • Develop schedules to assure that all programs have appropriate staffing levels.
Communicable Disease Coordinator

  • Provide case oversight in the immunization surveillance, investigation and outbreak prevention and control.
  • Provides consultation on a variety of community members including providers, schools, and the general public.
  • Collects and interprets reposted clinical data to assess need for treatment and follow -up.
  • Provides guidance to Community Health Workers

PHYSICAL REQUIREMENTS
This position requires potentially long periods of continued walking, standing, stooping, climbing stairs, sitting, bending, pulling, pushing, lifting up to approximately 30 lbs and two handed dexterity to examine and treat children and adults; May occasionally be exposed to unstable and dysfunctional family settings; communicable diseases and blood and other bodily fluids. Required to work in office conditions and travels throughout the community to work in clinical settings, homes, and other locations where services are delivered in high-risk neighborhoods and poorly maintained homes where rodents, animals, and various kinds of filth may be present; climb stairs to access clients in their homes.

EXPERIENCE
Level I-Grade 26:

A minimum of two (2) years of public health nursing experience.
Level II-Grade 27:

A minimum of one (1) year at LEVEL I with demonstrated experience and satisfactory job performance.
