Under the supervision of a higher classified employee, the Lead Community Health Worker (LCHW) will provide day-to-day guidance to Community Health Workers (CHW) to ensure and compliance with program guidelines. The LCHW will also provide a variety of outreach and customer service activities in both an office and community-based setting. LCHW, in partnership with the other CHWs, will build strong, mutually-beneficial relationships with community members. These connections can increase access to available health services and can support program and service enhancements where needed to better support community health for all. Additionally, position may provide support to all divisions within the Health Department, such as acting as a Health Department liaison in a variety of community settings.
This position is currently grant funded with federal, American Rescue Plan Act, funding for one year. It is anticipated that additional funding will be received to fund the position through 12/3/2024. Bi-lingual applicants are encouraged to apply.

Essential Duties:

  • Provides mentorship and day-to-day guidance to other CHWs in the Health Department.
  • Provides client-centered support for Washtenaw County Health Department programs and services in a community, home or clinical setting. Some examples may include: community lead testing, direct observed therapy, program enrollment, translation or other support as needed.
  • Independent judgment often required while working with other CHWs to support them and coach them in developing and implementing case management goals while including client voice and preferences in planning.
  • Leads CHW team meetings.
  • Analyzes data from other CHWs and interprets it in order to improve Health Department programs
  • Participates in the planning and implementation of client care plans.
  • May complete home or community visits, alone or with other staff, to provide services or engage with community members.
  • May teach other CHWs skills needed to increase independence in the areas of work skills, safety skills, social skills, finding and maintaining housing, and receiving and maintaining benefits.
  • Provides support to other CHWs who are working to secure needed services for the clients from any health, mental health, or social service provider. This may include giving advice about referrals and advocating for systems changes that better support clients┐ health or needs.
  • Provides community education and outreach for health department services.
  • Develops relationships with community partners to support health and the values outlined in the department's strategic plan.
  • Provides services in a culturally competent manner. Examples may include: providing written and spoken materials in clients preferred language and utilization of interpretation services.
  • Participates in individual, program and department processes with the goal of continuous quality improvement.
  • Provides timely and accurate documentation for services provided.
  • Updates and maintains a list of community services and agencies for referral purposes.
  • Performs related duties as assigned.

Knowledge, Skills and Abilities:

  • Ability to understand and apply an array of department and county policies, procedures, rules and regulations.
  • Ability to organize, prioritize and coordinate work with minimal supervision.
  • Ability to document interactions using written communication with clear English and proper grammar, spelling and punctuation.
  • Ability to use a wide variety of computer software including, but not limited to, Microsoft Office products, Outlook and social media such as Facebook and Twitter. May receive additional training on state and department required systems.
  • Ability to work cooperatively in a team and/or community setting while establishing and maintaining effective working relationships.
  • Ability to concentrate on client or coworker needs and support them in goal-setting.
  • Ability to provide quality client centered services under changing and challenging circumstances in an accepting and non-judgmental way.

  • Ability to maintain client confidentiality.
Proficiency in a second language.
Community Health Worker Certification recognized by the State of Michigan.
Strong community connections and/or similar life experiences to the community or population served.
Experience working with people.
Possession of a valid Michigan vehicle operator's license. Availability of automobile, van, or truck.
Duties require sufficient mobility to work in a both a normal office setting and in the community. Occasional lifting of up to 20 pounds may be required. Use of standard office equipment including specialized devices to communicate with hearing and speech impaired clients. Requires ability to read printed material and must be hearing and speech sufficient to communicate in person or over the phone. Requires ability to drive to multiple work locations. Must be willing to do home visits.

Possession of a BA or its equivalent. For equivalent status in lieu of education, at least two years must be as a certified community health worker as recognized by the State of Michigan.
At least 2-1/2 years as a Community Health Worker or similar work experience.
