JOB SUMMARY
|
|
Under the supervision of a higher classified employee, the Social Worker is responsible for making assessments, developing and implementing care plans, and evaluating appropriate records to assess potential need for social worker services.
|
|

EXAMPLES OF DUTIES
|
Essential Duties:
|
-
Provides client assessment of psycho/social needs in a clinic, office or home visit setting on receipt of referral for services.
|
-
Develops individual care plans within the context of the family environment which may include family members, significant others, supports and non-supports. Works with members of the project staff to manage care and coordinate with appropriate medical providers, outreach staff and community agencies involved in delivering accessible services. Documents services given.
|

Works closely with the Michigan Department of Health and Human Services (MDHHS) to assure clients have completed necessary steps for all entitlement programs if appropriate.
|
-
Facilitates communication between the client and professional/paraprofessional staff, medical care providers and community contacts.
|
-
Functions as a client advocate.
|
-
Maintains timely and accurate documentation in accordance with the requirements provided by the MDHHS Maternal Infant Health Program (MIHP) and WCHD MIHP, including care plans and progress notes.
|

Develops and maintains working relationships with area resource agencies.
|
-
Works with and provides consultation to project team and other Health Division staff.
|
-
Assists with staff training.
|
-
Cooperates with other staff in the review and evaluation of services.
|
-
Ability to cope with stressful situations or encounters, manage multiple and sometimes conflicting priorities and to work regularly with difficult medical/emotional psychosocial problems.
|

Ability to communicate information in a culturally competent way to influence behavior and impact health.
|
-
Use of Language line and other translation services.
|
-
Performs related duties as assigned.
|

LICENSES/CERTIFICATIONS
|
|
Possession of a valid Michigan vehicle operator's license. Availability of automobile, van, or truck.
|
|
PHYSICAL DEMANDS
|
|
Duties require sufficient mobility to work in a normal office setting and use standard office equipment including a computer, vision to read printed materials and a computer monitor and hearing and speech sufficient to communicate in person or over the telephone.
|
|

EDUCATION
|
|
Possession of a Master's degree in Social Work
|
|
Current licensure as a Licensed Master's Social Worker in the State of Michigan
|
|
EXPERIENCE
|
|
Two (2) years of experience providing services to families in the Maternal Child Health field
|
|
Home Visiting experience preferred
|
|
Bilingual preferred
|
