JOB SUMMARY
Under the supervision of a qualified professional Health Services Supervisor is responsible for performing direct vocational skills training services to individuals with a developmental disability and/or a mental illness. Duties include providing vocational services to individuals in accordance with Individual Plan of Service (IPOS) as developed by professional personnel including; skill building, supported employment, vocational training, and community integration. The Skills Trainer is further responsible for directly assisting individuals in achieving desired outcomes and documenting progress.
EXAMPLES OF DUTIES
Essential Duties:

  • Provides direct support to consumers with developmental disabilities and/or mental illness following interventions and supports outlined in the IPOS.
  • Assists consumers with improvement of daily living skills, providing personal hygiene, toileting, hand washing, feeding, showering as needed, and other skills as specified in the consumer's IPOS.
  • Teaches simple and complex skills needed to increase independent functions in the areas of work skills, safety skills, social skills, and oral comprehension skills.
  • Intervenes in the event of any aggressive or disruptive behavior as outlined in the individual's behavior plan.
  • Participates in the planning and implementation of IPOS.
  • Identifies and addresses health & safety or any other job related issues that need to be addressed in the Person Centered Planning process. Includes individual's strengths and desires in person centered planning.
  • Develops weekly agendas for assigned group
  • May need to be available to provide job coaching duties outside of normal business hours to accommodate success in a particular job site.
  • Documents services including; progress notes, behavior logs, incident reports, time studies, disabled for work preformed summary sheets, competitive employment statements, run sheets, vehicle inspection sheets and other reports associated with the work.
  • Ongoing, work-based vocational assessment which includes analysis of skills, strengths, preferences, prevocational work experiences, competitive work experiences as well as analysis of job loss (or job problems) as well as consistently updated with information from each new job experience.
  • Implement feeding guidelines, nursing guidelines, behavior guidelines and other guidelines as developed by professional disciplines.
  • Completes an Employment Plan and Vocational Assessments as needed for assigned consumers with assistance, review and signature of supervisor as needed.
  • Under direction of a supervisor and/or in consultation with the CMH Finance/Administration develops paid work, volunteer work and/or volunteer sites in the community for groups of individuals with developmental disabilities and/or mental illness.
  • Communicates and collaborates with a consumer's network of care, including their clinical team, family, and natural supports.
  • Represents the agency to the community, relatives and other interested persons in a manner that fosters understanding of the mental health needs of consumers, and the goals/mission of the organization.
  • Provides job development, training, and/or job coaching services
  • Provides individualized types of support that are based on the job, individual preferences, work history, needs, etc. Offers help with overall career development i.e. assistance with education, a more desirable job, or more preferred job duties.
  • Supports consumers in the development of community connections and relationships
  • Cultural sensitivity and awareness of the role of culture in the delivery of services
  • Provides consumers with transportation to various programming and volunteer sites in the community using Washtenaw County vehicles.
  • Use personal vehicle to meet groups at various sites/assignments throughout Washtenaw County.
  • As required, order supplies as necessitated by employment contracts keeping within the approved budget.
  • Administers medications and document administration of medications according to agency policy and after successful completion of medication training requirements.
  • Performs other duties as assigned by the Health Services Supervisor

PHYSICAL DEMANDS
  • Duties require sufficient mobility to work in a normal office setting and use standard office equipment including a computer, vision to read printed materials and a computer monitor, hearing and speech sufficient to communicate in person or over the telephone, and the ability to have a CDL.
  • Duties may require employee to exert/lift 50 or more pounds of force occasionally, 20 to 50 pounds of force frequently, or 10 to 20 pounds of force constantly.
  • Duties may require working in a confined space, bending, stooping, standing for extended periods of time, a moderate to extensive amount of walking, working outdoors during all weather conditions, and use of adaptive equipment.

Within the first six months of hire the employee will be expected to demonstrated competence in the following:

Knowledge and Understanding of:

  • Michigan Mental Health Code including Recipient Rights and Confidentiality provisions
  • Common developmental disorders and multiple diagnoses including chronic mental illness or substance abuse
  • Common health problems in adults with developmental disabilities
  • Person Centered Planning process
  • Medication and assistive devises commonly used to treat and or support adult with physical health and behavioral needs
  • Interventions and support needs of adults with multiple, chronic and serious health problems and behavioral needs
  • Community resources for adults with developmental disabilities
Skills in:

  • All phases of employment service including engagement, comprehensive assessment and individualized employment planning, job coaching and follow along support.

LICENSES AND CERTIFICATIONS
  • Possession of a valid Commercial Driver's License (CDL), Group A, B, or C with a "P" endorsement.
  • Applicants must meet any additional requirements made by funders for transportation of consumers.
EDUCATION
  • Possession of a high school diploma or its equivalent.
EXPERIENCE
  • Six (6) months prior experience working with adults with developmental disabilities and/or mental illness preferred.
