JOB SUMMARY
The nurse case manager will assist consumers with diabetes toward self-management of co-morbid diabetes and mental illness. The nurse case manager is a nurse who can perform a range of direct and indirect services within the Diabetes Disease Management program. He/she assesses the need for medical assistance, takes clinical histories, and identifies abnormal findings. In addition, the employee assesses dysfunctional behavior and emotional trauma and develops and carries out appropriate treatment plans.
The nurse case manager will be a member of the Health and Wellness team that supports consumers with the management of chronic health conditions in partnership with their mental illness. The medical case manager is a Registered nurse who can perform a range of direct and indirect services within the Health and Wellness program. He/she assesses the need for medical assistance, takes clinical histories, and identifies abnormal findings, provides coordination of care, health education and referrals/enrollment in health programs. This position will also be developing health related goals that are included in the treatment plan at WCCMH. This position also works closely with the treatment teams throughout WCCMH.
EXAMPLES OF DUTIES
Essential Duties:

  • Helps to maintain a Diabetes Registry
  • Takes clinical histories, identifies abnormal findings and assesses the need for medical assistance of identified consumers. Refers consumers to a behavioral health or physical health physician when appropriate.
  • Establishes nursing care objectives based on an assessment of a consumer's needs in relation to treatment goals
  • Assesses impact of co-morbid medical, psychiatric, and substance abuse diagnoses on management of diabetes. Assesses consumer need for assistance in managing diabetes. Assesses dysfunctional behavior, emotional trauma, and health status and develops and carries out appropriate treatment plans.
  • Develops a care plan for consumers identified as needing assistance in managing health conditions
  • Functions as part of the Disease Management team
  • Directly assists consumers in managing his/her diabetes (i.e., may assist consumers in checking blood glucose, attend medical appointments with the consumer, run a walking group, help consumer maintain a home blood glucose log, etc.) and mental illness
  • May provide mental health services to consumers in emergency situations. Identifies problem, precipitants, and resources available to the consumer to resolve the crisis.
  • Documents consumer status in electronic medical record
  • Communicates care plan to primary health care provider
  • Assists consumer in completing various documents (i.e., releases, referral forms, PAPs, etc.) as required or requested
  • Provides diabetes counseling, in conjunction with Health Educator or Case Manager
  • Coordinates care with CMH mental health team and CMH prescriber and participates in Person Centered planning process
  • Participate in daily team meetings (i.e., staff conferences) to assess and evaluate consumer's needs
  • Prepares forms, records, and reports associated with the work
  • Completes other duties as assigned

EMPLOYMENT QUALIFICATIONS:
EDUCATION
  • Possession of a Bachelor's of Science in nursing.
EXPERIENCE
  • A minimum of one (1) year of nursing experience, or the equivalent.
  • Six-month break-in period.
NECESSARY SPECIAL REQUIREMENTS
  • Registered professional nurse
