JOB SUMMARY
|
The nurse case manager with assist clients toward self-management of co-morbid physical and mental health conditions. The nurse case manager is a nurse who can perform a range of direct and indirect services within the Health & Wellness program. The nurse case manager assesses the need for medical assistance, takes clinical histories, creates care plans and identifies abnormal findings. In addition, the nurse assesses dysfunctional behavior and emotional trauma and develops and carries out appropriate treatment plans.

EXAMPLES OF DUTIES
|
Essential Duties:
|

Takes clinical histories, identifies abnormal findings, and assesses the need for medical assistance of identified clients. Refers clients to behavioral health or physical health physicians when appropriate.

Establishes nursing care objectives based on an assessment of a client¿s needs in relation to the client¿s health goals.
|

Assesses impact of co-morbidities, medical, psychiatric, and substance abuse diagnoses on management of physical health conditions. Assesses client need for assistance in managing disease. Assesses dysfunctional behavior, emotional trauma, and health status and develops and carries out appropriate treatment plans.

Develops a care plan for clients identified as needing assistance in managing physical health conditions.
|

Functions as part of the Health & Wellness team and community partners.
|

Directly assists clients in managing his/her conditions, including mental illness (i.e., teach clients to check blood glucose, etc., attend medical appointments with the client to advocate, facilitate groups, help client maintain a home monitoring logs, etc.).

May provide mental health services to clients in emergency situations. Identifies problem, precipitants, and resources available to the client to resolve the crisis.

Documents in electronic medical records. Prepares forms, records, and reports associated with the work.
|

Communicates care between CMH and healthcare providers within the community.
|

Assists client in completing various tasks (i.e., releases, referral follow up, medical equipment or supplies, etc.) as required or requested.
|

Provides coordination, in conjunction with Health Educator or Case Manager
|

Coordinates care with CMH mental health team and CMH prescriber and participates in Person Centered Planning process.
|
-
Participates in team meetings, attends, and facilitates conferences with community partners, and meet with insurance companies to assess and evaluate client¿s needs.
|

Completes other duties as assigned

EMPLOYMENT QUALIFICATIONS:

Knowledge, Skills, and Abilities:

Knowledge of chronic disease management and coordination.

Clinical assessment and writing skills.
|

Knowledge of the use of DSM-V
|

Knowledge of the common signs and symptoms specific to the target populations assigned.
|

Knowledge of the Michigan Mental Health Code including Recipient Rights and Confidentiality provisions.
|

Applies theory and scope of practice and Michigan Mental Health Code to professional practice.
|

Using sound independent judgement within established policy and procedural guidelines.
|

Ability to apply ethical principles.

Ability to follow organizational policies and procedures.

LICENSES/CERTIFICATIONS
|
Registered professional nurse license to practice in the State of Michigan.
|
PHYSICAL DEMANDS
|
Duties require sufficient mobility to work in a normal office setting and use standard office equipment including a computer, vision to read printed materials and a computer screen and hearing and speech sufficient to communicate in person or over the telephone. Duties also require mobility to attend community meetings.
|

EDUCATION
|

Equivalent to possession of a Bachelor of Science in nursing.
|
EXPERIENCE
|

A minimum of one (1) year of nursing experience, or the equivalent. Six-month probationary period.
|
