JOB SUMMARY
Under the supervision of a higher classified employee, the Social Worker is responsible for making assessments, developing and implementing care plans, and evaluating appropriate records to assess potential need for additional resources.

EXAMPLES OF DUTIES
Essential Duties:

  • Provides client assessment of psycho/social needs in a clinic, office or home visit setting on receipt of referral for services.
  • Develops client-centered comprehensive Plans of Care (POC) within the context of the family environment, which may include family members, significant others and supports as identified by the client. Works with members of the program staff to manage care and coordinate with appropriate medical providers, outreach staff and community agencies involved in delivering accessible services.
  • Monitors and coordinates service delivery to client/families as specified in the POC and ensures that it is client-centered and family driven.
  • Develops a supportive relationship with the client/family and works with them to promote successful outcomes specified in the POC.
  • Assists client/families in identifying and linking to needed and entitlement programs available in the community.
  • Coordinates and collaborates with the maternal child health network of care, including but not limited to, professional/paraprofessional staff, medical care providers, agencies, family and natural supports.
  • Assists in systems advocacy to secure needed services and resources either internal to the Washtenaw County Health Department (WCHD) or external community system service providers (eg. Medical providers, Michigan Department of Health and Human Services, etc).
  • Works with families to manage client's immediate crisis situation whether directly or indirectly by accessing needed services.
  • Provides support and education regarding various topics including, but not limited to, the stages of pregnancy, labor and delivery, breastfeeding, pregnancy complications, family planning, substance misuse, nutrition, maternal mental health, the impact of trauma, overall infant growth and development, attachment and infant feeding.
  • Maintains timely and accurate documentation in the electronic medical record in accordance with the requirements provided by the MDHHS Maternal Infant Health Program (MIHP) and WCHD MIHP, including consents, plans of care progress notes, client/family contacts, Ages and Stages Development Screenings and MCIRs to meet certification requirements.
  • Develops and maintains working relationships with area resource agencies.
  • Works with and provides consultation to program team and other Health Division Department staff and external partners.
  • Assists with staff training.
  • Cooperates with other staff in the review and evaluation of services.
  • Ability to cope with stressful situations or encounters, often in the home environment or a community setting.
  • Ability to manage multiple and sometimes conflicting priorities and to work regularly with client's experiencing difficult medical/emotional psychosocial problems.
  • Communicates information in a culturally competent way including the ability to transform knowledge and cultural awareness into interventions that influence behavior and impact health.
  • Ability to effectively use the language line and other translation services in service delivery.
  • Performs related duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES
Upon hire, applicant has basic required competencies that include:

  • Knowledge of available community resources and supports for pregnant women and infants.
  • Knowledge of the Child Protection Law
  • Ability to use sound judgement and practice autonomous decision making, seeking supervision as needed.
  • Ability to represent the agency to the community, natural supports and other interested persons in a manner that fosters understanding of the needs of the pregnant individual/infant, and the goals and mission of the organization.
  • Ability to assess the safety of pregnant individuals, infants and self as a professional services provider.
  • Ability to manage crisis situations and seek support appropriately.
Within the first six months of hire, employee will be expected to deliver services demonstrating competence in the following.

  • Knowledge about current trends for the maternal child health population by completing the MDHHS MIHP New Professional Staff training within 30 calendar days from the date of hire.
  • Knowledge of the Public Health Code
  • Expanded knowledge of community resources
  • Ability to work both autonomously and as a member of a multidisciplinary team and to make use of supervision in the development of skills and competent practice.
  • Ability to communicate effectively with clients, family members, natural supports and community agency staff.
  • Ability to effectively prioritize work.
  • Increased understanding of evidenced based practices for working with the maternal child health population and home visiting.
  • Willingness to actively engage in the professional development process
  • Knowledge of how to access agency policies and procedures.
  • Demonstrated skill and knowledge of evaluation/assessment of client/family service needs.
  • Demonstrated knowledge of Medicaid covered services
  • Demonstrated knowledge of administration and use of ASQ-3 and ASQ-SE through age 18 months.
  • Demonstrated skill and knowledge of WCHD services and supports and other contractual services.
  • Demonstrated proficiency with using computers adequate enough to use electronic medical record and mobile technology.
  • Demonstrated skill in the ability to provide documentation according to agency policy. Seeks supervisory support and direction for clarification as needed.
  • Demonstrated increased proficiency in working with non-English speaking clients/families and use of language services for service delivery.

LICENSES/CERTIFICATIONS
Possession of a valid Michigan vehicle operator's license. Availability of automobile, van, or truck.
PHYSICAL DEMANDS
While performing the duties of this job, the employee is regularly required to stand, walk, sit, talk, see and hear. The employee is occasionally required to stoop, kneel, crouch, crawl and reach with hands and arms. The employee must occasionally lift and/or move up to 30 pounds such as pack and plays. Specific vision abilities required by this job include close vision, distant vision and the ability to adjust focus. The employee is required to have hearing and speech sufficient to communicate in person or over the telephone. This position requires the individual to travel and/or drive to various off-site locations. In addition, this position requires the individual to sometimes work irregular or extended work hours and meet multiple demands from several people.

EDUCATION
Possession of a Master's degree in Social Work
Current licensure as a Licensed Master's Social Worker in the State of Michigan
EXPERIENCE
Two (2) years of experience providing services to families in the Maternal Child Health field
Home Visiting experience preferred
Bilingual preferred
