JOB SUMMARY
This is the full functioning professional level performing a range of direct and indirect services within a specific program. The employee assesses the need for medical assistance, takes clinical histories and identifies abnormal findings. In addition, the employee assesses dysfunctional behavior and emotional trauma and develops and carries out appropriate treatment plans, while also providing medical support services to individuals with a developmental disability and/or mental illness receiving services from WCCMH. This position could also provide the following services: comprehensive care management, health education, manages and provides services to those with chronic health conditions.
EXAMPLES OF DUTIES
ESSENTIAL DUTIES:

  • Provides patient care using the nursing process interdependently under the direction and supervision of a RN.
  • Collaborates with other medical staff in an interdependent role as a health care team member.
  • Provides medical care and therapies to all clients assigned in a dependent role.
  • Acts as a patient advocate in health maintenance and clinical care to ensure the best possible care for all clients.
  • Assists in performing nursing assessment: Performs focused screening/assessments/ observational assessments; Initial, admission, or event-focused assessments; Collects data and information; Recognizes relation to diagnosis
  • Communicates outcomes of the data collection process in a timely manner to appropriate medical staff for continuation of treatment.
  • Determines immediate need for intervention depending on the situation and severity of client¿s needs.
  • Provides data to assist in development nursing diagnosis.
  • Contributes to the development of the nursing care plan and assists in developing expected outcomes.
  • Implements the identified nursing care plan under RN supervision.
  • Delegates and supervises certain nursing tasks to UAP (unlicensed assistive personnel) in specific situations and settings to allow for integrated services and appropriate care such as nursing care plan activities and medical regimens.
  • Assists in coordinating care and identifying case management needs with other team members for all clients within the WCCMH system.
  • Assists in patient teaching to encourage successful exit of care.
  • Contributes to evaluation by reporting patient outcomes and documents in the medical record.
  • Communicates (verbally or in writing) with RN and other health care practitioners/health care team as appropriate.
  • Refers patients to appropriate resources based on Individual Plan of Service and medical needs within and outside of the WCCMH system.
  • Carries out medical regimens under the direction and supervision of an RN or other authorized health care practitioner.
  • Performs basic nursing care in routine situations for patients.
  • Participates in developing, providing, and evaluating health education, health promotion and preventive activities to patients, families, and groups under the direction and supervision of an RN or other authorized health care practitioner.
  • Under direction of an authorized health care practitioner may give medications, treatments, tests, injections, vaccines (whether piercing or puncturing of tissues is involved and whether a degree of independent judgement and skill is required).
  • Responsible and accountable for the quality of nursing care given.
  • Responsible and accountable for following the standards of nursing conduct or practice.
  • May only perform activities within the LPN's individual scope of practice.
  • Performs other duties as assigned.

EMPLOYMENT QUALIFICATIONS:
Knowledge, Skills, And Abilities:

  • Knowledge of the Michigan Mental Health Code including Recipient Rights and Confidentiality provisions.
  • Knowledge of common developmental disorders and multiple diagnoses including chronic mental illness or substance abuse.
  • Interventions and support needs of adults and youth with multiple, chronic, and serious health problems and behavioral needs.
LICENSES AND CERTIFICATIONS:
Current license as a Licensed Practical Nurse - LPN in Michigan.
PHYSICAL DEMANDS:
Duties require sufficient mobility to work in a normal office setting and use standard office equipment including a computer, vision to read printed materials, a computer monitor, and hearing and speech sufficient to communicate in person or over the telephone.
Duties may require employee to exert/lift 50 or more pounds of force occasionally, 20 to 50 pounds of force frequently, or 10 to 20 pounds of force constantly. Duties may require working in a confined space, bending, stooping, standing for extended periods of time, a moderate to extensive amount of walking, working outdoors during all weather conditions, and use of adaptive equipment.

EDUCATION
Practical nursing diploma required. After graduating with your diploma in practical nursing, you must pass the National Council Licensing Examination for Practical Nurses exam (NCLEX-PN) to gain the LPN licensing to practice. Advance degree preferred.
EXPERIENCE
One (1) year of nursing experience. Six-month break-in period.
