JOB SUMMARY
|
This is the full functioning professional level completing comprehensive medical/physical evaluations as part of the OBRA Level II comprehensive screening evaluation for the federally mandated OBRA Program. The employee assesses the medical and surgical status of people considering admission to a skilled nursing facility who also have mental health needs. The nurse takes clinical histories, identifies abnormal findings, and makes recommendations for follow up. In addition, the employee assesses dysfunctional behavior and emotional trauma to determine if mental health needs can be managed in a nursing facility enviornment.

EXAMPLES OF DUTIES

Essential Duties:
|

Records medication and treatment noting and evaluating any side effects.
|

Takes clinical histories, identifies abnormal findings and assesses level of care needs. Makes recommendations for follow up.
|

Establishes nursing care objectives based on an assessment of a client's needs in relation to treatment goals.
|

Assesses dysfunctional behavior, emotional trauma, and health status and develops recommendations for appropriate treatment.
|

Identifies problems, intentions of the client and resources available to determine the appropriate level of care. Makes appropriate case dispositions.

Attends staff conferences to assess and evaluate client's needs.
|

Completes comprehensive assessments based on the specific requirements in the DCH OBRA Operations Manual.

Performs other duties as assigned by the Program Supervisor.

EMPLOYMENT QUALIFICATIONS

PHYSICAL DEMANDS
|

Duties require sufficient mobility to work in a normal office setting and use standard office equipment including a computer, vision to read printed materials and a video display terminal (VDT) screen and hearing and speech sufficient to communicate in person or over the telephone. Duties also require mobility to complete assessments in acute care hospitals, nursing facilities, private homes, group homes, or correctional facilities.

EDUCATION
|

Possession of a Bachelor's of science degree in nursing.
|
EXPERIENCE
|

One (1) year of nursing experience.
|

Six-month probationary period.
|
NECESSARY SPECIAL REQUIREMENTS
|

Registered professional nurse
|

Possession of a valid Michigan Driver's License is required.
|
